Capital Health earns highest rating from Accreditation Canada

Capital Health is thrilled to share the news that following a review of our evidence submissions, we have earned the highest rating from Accreditation Canada - Accredited with Exemplary Standing. Accreditation with Exemplary Standing is granted to those organizations which exceed the most rigorous requirements of the accreditation program  so this is a significant accomplishment.

Accreditation Canada congratulated “leadership, staff and accreditation team members for their efforts and dedication to the provision of safe, quality health services.”

This rating was based on a week-long survey of Capital Health in October 2013 as well as evidence we submitted to Accreditation Canada up to the end of March 2014.

Accreditation in and of itself is not the reason we strive to improve - we do it because it’s the right thing to do for our patients, our clients and ourselves. However, accreditation is a useful tool for measuring our progress and provides information that enables us to improve further.

As the surveyors noted, the results of the accreditation report demonstrate that we are living Our Promise in Action every day. We are well on our way to becoming a world-leading haven for people-centred health, healing and learning. In remaining focused on continuous improvement, we get ever closer to this goal.

Throughout the survey week, surveyors were impressed by many things, including:

  • The alignment between our strategic plan, Our Promise in Action, and our day-to-day actions
  • The multi-year business planning process
  • The My Leadership program
  • Innovative communications
  • Community focus
  • Actively engaged employees and physicians

In addition to the advanced Accreditation standing, Capital Health has received an outstanding eight leading practices to date:

  • Frailty Assessment for Care Planning Tool (FACT)
  • STEP: Standardized Team Education Program & CoCGA: Collaborative Comprehensive Geriatric Assessment
  • Community Cardiovascular Hearts in Motion
  • Community Health Teams Citizen& Stakeholder Engagement
  • The Community Health - Health & Wellness Program Framework
  • Community Health Team - Low Intensity Exercise Program
  • Implementing a Novel and Supportive Program of Individualized Care for Patients and Families Living with Respiratory Disease (INSPIRED)
  • Patient Initiated Family Screening Program for Hereditary Aneurysm Syndromes
