Changes to incident investigation report forms to make process easier and more efficient

Safety and Injury Prevention, Central Zone, has made some changes to incident investigation report forms to make the process easier and more efficient. There are now two different incident investigation report forms, one for violence and one for hazards and incidents. Both forms are now available to be filled out and saved online. These new forms will be in the email link you receive from the safety response officers when a staff member calls SAFE to report an incident. They are also available on our Safety and Injury Prevention website. These reports will still need to be distributed to the same people, affected employee, SAFE and the applicable Joint Occupational Health and Safety Committee.
