Educational Institution/Employer

The role of the educational institution/employer is to assist their students in preparing for their placement at Capital Health and ensure all of the required documentation is received by Student/Learner Placement Service by the deadlines provided.

Responsibilities

  • Signs Capital Health’s Affiliation Agreement (A placement search will not be initiated until the signed Affiliation Agreement is received.)
  • Submits a completed placement request form (Student or Employee)
  • Confirms that a student / employee has provided proof of all the required immunizations outlined in Capital Health’s Pre-placement Immunization Requirements CH 100-026 policy. If you are not familiar with interpreting immunization records, you may wish to have your students/employees submit the Proof of Required Immunization form (completed by a health professional) to you, rather than their personal immunization records, to assist you in ensuring that they meet Capital Health's requirements.
  • Conveys the results of a criminal records check, including a vulnerable sector search, performed as a pre-requisite for enrollment in the program, employment, or within the past six months as outlined in Capital Health’s Criminal Records Check CH 40-150 policy

Do not send us a student’s or employee’s personal immunization records or criminal record check results form as it is the institution’s responsibility to keep these documents in their files. Immunization confirmations and Criminal Records Check results must be submitted via the on-line form at least two weeks before placement begins.
