• Complete Our New Electronic Form - We welcome your questions and inquiries by phone. Effective Monday, April 6, 2015, the Regional Tissue Bank will no longer accept fax/phone tissue orders. This Quality Initiative will enhance order accuracy and communication with our clients. Download Tissue Request form.
• Send Completed Form - Purchase Order Requisitions may also be attached to the email. Send completed form to tissuebank@cdha.nshealth.ca
• Watch Your Inbox for Order Process - One or more email addresses you provide will be notified by Customer Service when your order is received, processed and then shipped.
